JOB DESCRIPTION
Job Title: Team
Leader
Hours of Work: Nine days on average, across
fortnightly rota. You will need to be able to
work
a flexible rota pattern to cover unsociable hours.
Pay rates: £15,366
pro rata plus enhancements.
Accountable to: On a day to day
basis the Team Leader will take instructions from the their direct line Care
Coordinator, others senior staff who may be covering in their absence and the
Registered Manager.
Purpose of job: To support a
team of Care Attendent who work in a geographical area of Sandwell Crossroad’s
defined area of care. To
facilitate with your team of Care Attendents a variety of client care needs specifically
in the first instance to support Carers to take a break from caring.
1)
To follow a prescribed care plan; this will involve helping the service
user regain confidence, safety and independence with e.g.. getting in/out of
bed, washing/bathing, dressing, preparing breakfast/lunch/tea/supper,
toileting, managing incontinence, walking in/outdoors, ascending/descending
stairs, cleaning, shopping, laundry.
2)
To follow a prescribed programme of nursing assistance application of
simple repeat dressings, skin care, medication, eye drops, ear drops,
nebulisers, catheter care, stoma care, sheath care, prostheses etc. with help
being tailored to the individual needs, willingness to participate,
capabilities and confidence of the service user.
3)
To follow a prescribed programme of support to people suffering from
dementia including for example,
developing memory prompts, diaries etc. daily routines, maintaining the
environment etc., to help service users follow as normal a pattern of daily
activities as possible, with help being tailored to the individual
needs, willingness to participate, capabilities and confidence of the service
user.

To follow a prescribed pattern of exercises when necessary under the
strict supervision of a Physio Thearpist overseeing the client care, who
provides regular support to the Service User.
5)
To visit service users in their
own homes - by driving across the
relevant part of the geographical area of Sandwell.
6)
To produce staff rota/time sheets information either in written or
computrised format to the Care Co-ordinator
on a weekly basis, with anticipated changes and client care cover.
7)
Checking and verify staff timesheets on a weekly basis.
8)
Supervising staff on an individual basis and holding team meetings as
necessary to aid communication within the team. Supporting staff through their
induction period and the SCILS for Care Induction Standards.
9)
To undertake the first visit of a client care programme to ensure the
health and safety assessment is correct before handing the package of care over
to the Care Attendent.
10)
When on duty to be on call for your team and your co-Team Leader covering
their area of work in their absence.
11)
Cover for staff sickness by either amalgamating the hours into your own
rota or covering the call with another team member.

Record Keeping – To keep appropriate daily client records in the approved
form and to help prepare reports for reviews/ case conferences. To complete and
submit expenses claim forms and time sheets of your own and check staff time
sheets on a weekly basis.

To take part in, and contribute to regular reviews/case conferences to
support the Carer and Service User liaising with family, other Health and
Social Work professionals.

To attend the weekly Meetings with the Care Co-ordinator to provide
valuable feedback on each service user to help the team review progress and
plan for the following weeks rota.
15)
To consult the Care Co-ordinator whenever there is concern/
deterioration, significant change, unusual events, changes in social
circumstances. Also to update the relevant
team members on relevant progress of the Service Users whom they may be
covering.
16)
Health and Safety – Before a service is provided, there will be an
assessment of Health and Safety factors from the Team Leader viewpoint as well
as a Risk Assessment of any risks to
the service user. The Team Leader is
required to follow health and safety requirements generally and specifically to
follow the individual assessments. Each
Care Attendent will be provided with confidential written information about the
service users medical and social history.

Moving and Handling (current training certificate required).

Code of Practice for assistance with the use of medicines.

Food Hygiene (current certificate required).
·
To encourage Service Users, Relatives, Carers and significant others to
contribute to every aspect of the care plan to ensure that individuals are
enabled to make decisions about their own lives and to make a contribution to
the development of services in general.
18)
Other Duties
·
To promote the philosophy of "ordinary life" and independent
living according to the wishes of the Service User
·
To receive complaints and to help ensure that any complaints are fully
investigated in accordance with the Crossroads Care Complaints Procedures.
·
To attend training courses relevant as required.
·
Any other duties consistent with the general level of duties and
responsibilities of the job description.
·
To report any suspicions of abuse to the service user to the Registered
Manager in line with the Adult protection policy.
